Use it as a starting point and adapt it to suit your school's context.

The questionnaire should be used in lesson time, so the teacher can read the questions out loud, and help any pupils who need support. 

Younger pupils may need an adult to scribe for them. Pupils with special educational needs (SEN) and/or English as an additional language (EAL) may need additional support.
